SCH is a small community based FQHC facility in the heart of Syracuse, NY. - Residents serve vulnerable populations: uninsured, Medicaid, migrant workers, refugees, rural or inner-city populations. Strong emphasis on health equity, social determinants of health, and community advocacy. Residents focus on chronic disease management, preventive care, and behavioral health integration and - learn how to care for patients across language, cultural, and access barriers. -
